Ocean Street
Residential Treatment Program
The Ocean Street program recognizes the unique needs of each resident,
seeks creative ways to offer life-affirming and empowering experiences, and works to provide an environment where clients can maintain individuality and dignity, access community opportunities, and enhance their independence and quality of life.
The Ocean Street Residential Program, with a capacity of eight residents, provides trauma-informed treatment and support services to adults living with mental illness complicated by co-occurring chronic health conditions as well as a potential substance use disorder.
In a supportive, safe, and secure environment, the program meets the needs of individuals who have a major mental illness by providing psychosocial rehabilitation services and personal care services.
The services and supports provided are necessary for the residents with severe and persistent mental illness to remain in the community and maximize their potential for leading stable, pleasurable, and productive lives.
The program works to address the complex needs of the clients with an integrated, individualized, client-centered approach, led 24 hours a day by staff trained in trauma-informed care, mental health and substance use treatment, crisis intervention, and other best practices. To help individuals stabilize and develop goals that will move them toward the most independent and supportive living situation possible, staff and clients work together to develop individual care plans.
Program services and strategies include:
Supportive services in a residential setting
On-going care coordination
Mental health and substance use treatment
The development of independent living skills and a permanent housing plan
Referrals to community resources (e.g. primary care, mental health and substance use services, peer recovery programs, housing, support groups, financial support, educational services, and employment resources.)
POPULATION SERVED
Adults who have been diagnosed with a major mental illness, living with co-occurring chronic health conditions who may also have a substance use disorder.
