Residential Treatment Program
Gordon Green Residential Treatment Program, with a capacity of eight residents, provides trauma-informed treatment and support services to adults living with mental illness complicated by co-occurring chronic health conditions.
In a supportive, safe, and secure environment, the program meets the needs of individuals who have a major mental illness by providing psycho-social rehabilitation services and personal care services. The services and supports provided are necessary for the residents with severe and persistent mental illness to remain in the community and maximize their potential for leading stable, pleasurable, and productive lives.
At Gordon Green, adults with behavioral health and medical issues find a safe and secure home. We provide services that help our residents live to their fullest abilities and thrive. The program works to address the complex needs of the residents, utilizing an integrated, individualized, resident-centered approach, managed 24-hours-a-day by staff trained in trauma informed care, mental health treatment, crisis intervention, and other best practices.
Program services and strategies include:
Supportive services in a residential setting
Ongoing care coordination
Personal care
Meal preparation
Assistance with daily activities
Medical monitoring and medication administration
Mental health treatment
Transportation to recreational activities and appointments
Organized community outings
Referrals to community resources (e.g. primary care, mental health and substance use services, peer recovery programs, housing, support groups, financial support, educational services, and employment resources.)
POPULATION SERVED
Adults who have been diagnosed with a major mental illness, living with co-occurring chronic health conditions who are aging in place.
